Waiver

Consent, Liability Waiver, and Refund Policy

I hereby consent to have my minor child participate in the following activities at Coach Moe Athletics, LLC.

I acknowledge that there are risks involved in participating in this activity and hereby assume all risk of injury, harm, damage, or death to my minor in connection with his/her participation in this activity. I, for myself and my minor child, willingly assume the risks associated with participation and accept that there are also risks that may arise due to other participants. I agree that my minor child shall comply with all stated and customary terms, posted safety signs, rules, and verbal instructions as conditions for participation in any activity offered by the Company.

I agree to release, indemnify, defend, and hold the Company, its officers, directors, owners, affiliates, successors, assigns, and subsidiaries harmless from any loss, expense, liability, claim, or injury, including but not limited to attorney’s fees, arising out of my participation in training classes offered by Coach Moe Athletics LLC. Our release, indemnification, defense, and hold harmless obligations shall apply regardless of whether said accident, loss, liability, expense, claim, or injury is the result of negligence, recklessness, or otherwise, excluding only those incidents resulting directly from Coach Moe Athletics LLC's gross negligence or willful misconduct. We understand and agree we will be held financially responsible for all damages to property and/or equipment resulting from participation in the above, and we knowingly and voluntarily agree to assume such responsibility as a condition to the minor child’s participation in the training classes referenced herein.

Refund Policy

At Coach Moe Athletics, we strive to provide high-quality basketball training services. Refund requests must adhere to the following guidelines:

• Refund requests must be made in writing and submitted at least 14 days prior to the scheduled training session or program start date.

• Requests must include the client’s name, contact information, reason for the refund, and proof of purchase (if applicable).

• Refunds are granted at the discretion of Coach Moe Athletics management and are only approved for valid reasons, such as medical emergencies, relocation, or unforeseen circumstances beyond the client’s control.

• Refunds will not be provided for no-shows or cancellations made less than 14 days before the scheduled training session or program start date.

• Partial refunds may be granted based on the number of training sessions attended or services utilized before the request.

• Refund inquiries or requests can be submitted via email to coachmoeathletics@gmail.com.

By signing this form, I acknowledge and agree to the refund policy stated above.

I acknowledge that I have read this form thoroughly and in its entirety and fully understand that it is a release of liability, and I hereby execute this agreement without coercion. By signing this release, I am waiving any right I have to bring a legal action or claim of any kind against the Company, its officers, directors, owners, affiliates, successors, assigns, and subsidiaries as a result of participation in any program through the Company.

I understand that entry into Coach Moe Athletics LLC training facility and execution of this form constitutes consent for Coach Moe Athletics LLC to use any film, video, or likeness of participants for any purpose whatsoever, without payment to the participant.

The invalidity or unenforceability of any provision of this Agreement shall not affect the validity or enforceability of any other provision of this Agreement, which shall remain in full force and effect.
